What Is LAMS? Awake Liposuction Under Local Anaesthesia Explained

Key takeaway: LAMS (Local Anesthetic Minimal-Invasive Liposuction) is an awake liposuction technique introduced by 365mc in 2014. Only the area being treated is numbed, so you are conscious and able to talk throughout, and fat is drawn out through a very fine tube inserted through a tiny opening. It is designed for localised, small-to-moderate areas of fat and typically involves less downtime than conventional liposuction, although recovery is not instant.

Awake liposuction appeals to many international patients because it avoids general anaesthesia and can fit into a shorter trip. But "awake" does not mean "minor" in every respect. This guide explains how the procedure works, what you will feel, who it is suited to and what the limits are.

How does awake liposuction differ from conventional liposuction?

Both approaches remove subcutaneous fat, the soft layer beneath the skin. The main differences lie in anaesthesia, instrument size and scale.

  • Anaesthesia: awake liposuction uses local anaesthetic injected into the treatment area; conventional liposuction is usually done under general anaesthesia or sedation.
  • Scale: awake techniques suit smaller-to-moderate volumes; conventional surgery can treat larger volumes in one session.
  • Instruments: the LAMS cannula is narrow, between 0.3 and 0.6 cm across, and needs only a small puncture to enter.
  • Garments: no compression garment is required after LAMS, whereas conventional liposuction usually involves one.
  • Recovery: generally shorter after awake liposuction, but swelling and bruising can still occur.

The three stages of LAMS

LAMS follows a defined sequence that is designed to make extraction smoother and more even.

  1. Tunnelling. After the local anaesthetic takes effect, the doctor creates small channels through the fat to loosen the fibrous bands that hold fat in place. This makes the next stage more controlled.
  2. Extraction. The narrow cannula then draws the loosened fat out. You may feel pressure, movement or vibration, but the area should not feel sharp pain once numb.
  3. Dissolving solution. A solution is introduced to help clear remaining fat close to the treated area, supporting a smoother transition between treated and untreated tissue.

The technique was developed through joint clinical research with Seoul National University College of Medicine and received an Excellence Award at the KSDS International Conference in 2024.

What does it feel like to be awake during liposuction?

The initial local anaesthetic injections usually cause stinging or a burning sensation for a short time. Once the area is numb, most people describe pressure, pulling or a vibrating feeling rather than pain. Sensitivity varies between individuals and between body areas.

Being awake means you can talk to the medical team and tell them if you feel discomfort, so they can add anaesthetic where needed. Some people find it reassuring; others feel anxious. If you are nervous about being conscious, discuss this honestly at consultation, as it is an important part of deciding whether awake liposuction is right for you.

Who may awake liposuction suit, and who may it not?

It may suit you if

  • You have localised, stubborn fat in areas such as the arms, abdomen, flanks, thighs or under the chin.
  • You prefer to avoid general anaesthesia.
  • You have limited time off work or a shorter stay in Korea.
  • You are in good general health and close to a stable weight.

It may not suit you if

  • You want a large volume removed from many areas in one session; conventional liposuction or an extended option such as Infinite LAMS may be discussed instead.
  • Your concern is mainly loose skin rather than fat.
  • You expect significant weight loss. LAMS refines shape; keeping a healthy weight remains your job.
  • You have a condition, or take medication, that makes local anaesthesia or minor surgery unsafe.

Before anything is booked, a doctor looks at your health record, any medicines you take and where your fat actually sits.

Recovery after LAMS: realistic expectations

Many people walk out of the clinic the same day and return to light activities soon afterwards. That said, a shorter recovery is not the same as no recovery.

  • First 24 to 48 hours: tenderness and some fluid seepage from the entry point are common. Keep dressings clean and follow wound-care instructions.
  • First week: swelling and bruising may appear and gradually fade. Desk work is often possible within days, depending on the area and how you feel.
  • Weeks two to six: residual swelling settles; some firmness is normal while tissues heal.
  • Ongoing: the contour becomes clearer over several months.

After LAMS, 365mc uses aftercare such as shockwave therapy (ESWT) as part of its Orange Care programme. Leaving before the sessions are finished? You will be given a routine to follow at home. Read more on bruising and swelling after treatment.

Risks and side effects to know

Avoiding general anaesthesia removes some risks, but LAMS is still a medical procedure. Possible effects include:

  • Bruising, swelling, tenderness and temporary numbness
  • Firmness or small lumps during healing
  • Uneven contour or asymmetry
  • Infection at the entry point
  • Reactions to local anaesthetic, which are uncommon but possible

Tell your doctor about any blood-thinning medicines, supplements or weight-loss medications, as these can affect bruising and healing.

Planning awake liposuction in Seoul

  • Length of stay: many people plan around a week for a single area, allowing time for the procedure and early check-ups. Your doctor will advise you individually.
  • Flights: avoid flying immediately after the procedure if possible, and move around regularly and stay hydrated on long flights.
  • Clothing: bring loose, dark, comfortable clothes for the day of the procedure and the days after.
  • Follow-up: confirm how you will send photos or questions remotely once you are home.

Questions patients often ask

Is awake liposuction safer than general anaesthesia liposuction?

It avoids the specific risks of general anaesthesia, but no procedure is free of risk. Safety depends on the volume treated, the doctor's experience and your health.

How much fat can LAMS remove?

It is designed for small-to-moderate volumes. Your doctor sets a safe amount based on your body and the areas involved.

Will I need a compression garment after LAMS?

No compression garment is required after LAMS, which many people find makes travelling and recovery more comfortable.

Can I watch or talk during the procedure?

You can talk to the team throughout. Whether you can see the area depends on its position, and many people simply rest, listen to the staff or close their eyes.

How soon can I go back to work?

Many people return to desk work within a few days, but this varies with the area treated and your job. Physical work may need more time.
